The Sheffield Primary and Community Mental Health Service delivers interventions for adults with serious mental illnesses, in places near to their homes. We also want to provide support at the earliest possible stage, to prevent against deteriorating mental health.

The service is delivered by a multidisciplinary team of clinical psychologists, occupational therapists, mental health pharmacists, cognitive behavioural therapy (CBT) therapists, mental health practitioners, health coaches, peer support workers, mental health wellbeing practitioners and Sheffield Mind's Community Connectors, as well as voluntary sector organisations, who offer additional support and activities. 

Our Community Connectors work with these colleagues in the Primary Care Networks (PCN's) to ensure that people with severe mental illness get the most appropriate local support - and they support individuals to feel more able to use and be part of their local community. Working from our Wellbeing Centre and in their local PCN, they are helping people to access wider community support, by working with them to remove any barriers that might otherwise prevent them from making the most of that support.
