Community Connections

Sheffield has been selected to deliver a new place-based community mental health service for adults and older adults. £5.2 million has been invested in the scheme to develop and implement a transformation approach. The Primary & Community Mental Health Transformation Programme is a partnership between Sheffield Health and Social Care, Primary Care Sheffield, NHS Sheffield CCG and the voluntary community and social enterprise sector (VCSE).

The focus of the Transformation Programme is to develop support for people with serious mental illness living in four areas of the city. It provides access to psychological therapies, improved physical health care, personalised and trauma-informed care, medicine management and access to support and social activities.

The service is being delivered through new multidisciplinary team of Clinical Psychologists and Psychosocial Practitioners, Occupational Therapists, Mental Health Pharmacists, CBT Therapists, Primary Care Mental Health Practitioners and Community Connectors (the element of the project that is managed by Sheffield Mind).

Our Community Connectors offer support to people with serious mental illness, to access a range of support and social activities within their neighbourhood, with a view to them developing new skills and building their confidence.
